  • Outpatient MAT with Suboxone, Subutex, Sublocade, individual therapy, telehealth 

📝 2. What You'll Need to Do

  1. Pick a location based on your needs (in-person vs. telehealth, hours, insurance).

  2. Call or text using one of the provided numbers.

  3. Use a simple script like:

    “Hi, I’m [Your Name], and I’d like to start Suboxone treatment. Do you have new patient availability? I have [insurance/Medicaid/no insurance]. What’s the intake process and timeline?”

  4. Ask:

    • What forms or info you need to bring

    • Whether the first visit is in-person or telehealth

    • What to do about labs or urine screens

    • Insurance, Medicaid, sliding scale, and any costs

📆 3. What to Expect at Intake

  • A licensed provider (doctor or nurse practitioner) will:

    • Review your medical and opioid use history

    • Evaluate withdrawal level

    • Discuss buprenorphine (Suboxone) and its effects

    • Possibly order lab/urine tests

  • You may be prescribed Suboxone after initial evaluation

  • Case management, counseling, and follow-up plans set

✅ 4. Prep Checklist for Making the Call

  • Your name, DOB, contact info, and insurance status

  • A brief note like: “I’ve been using [drug] for [duration] and want to start Suboxone”

  • Ask about appointment availability & clinic hours
